Peter Ladkin is the developer of the formal and semi-formal failure and accident method Why-Because-Analysis (WBA), which has been used to analyse commercial aviation and railway accidents, as well as a friendly-fire accident and a cruise ship grounding, as well as computer security incidents. The group proposes Ontological Analysis, which incorporates causal system analysis, for safety requirements elicitation, specification and analysis.
